health matters Quality health care is important at Johnson & Wales. Providing Health Services with an accurate health record (such as any allergies you have, personal and family history, and emergency contact information) ensures that the staff in Health Services can provide you with the best service possible. As a prerequisite to registration, the University and local health department require that you submit proof of a physical exam that was conducted within the last year, including documented proof of immunity against measles and rubella. All medical information is confidential and cannot be released by University Health Services, according to federal law, without your written consent. health insurance Because of the high cost of health care in the United States, all international students are required to have health insurance. If you do not have an insurance policy, you will be required to purchase a plan from the University prior to registering for classes. If you do have an insurance plan, you must bring proof of insurance with you to registration so that you are not billed for the University plan. Students on F-1 visas may choose any insurance policy which is comparable to the University policy. Insurance is optional for dependents on F-2 visas, however, you are strongly advised to purchase an insurance policy. The University insurance plan does not offer coverage to spouses or the children of students. reservation deposit A $300 non-refundable reservation deposit is required in order to confirm your enrollment at Johnson & Wales. Please note that your schedule of classes will be prepared only upon receipt of this deposit. travel request forms All students are required to submit the travel request form, which is enclosed in the accepted student guide. Please be sure to let us know of your arrival plans so that a schedule of classes and accommodations can be prepared for you. Students who want to live on campus in the dorm must arrive two days prior to registration. |
